Symmetrical peripheral gangrene (SPG) is an uncommon clinical entity. It is caused by a vascular injury secondary to a disseminated intravascular coagulation (DIC) mechanism. Its onset is unpredictable, and there are currently no therapeutic options to prevent or reverse its progression. For this reason, in most cases its treatment is nothing more than the treatment of the sequelae, consisting of limb amputation. We present a review of this topic based on a clinical case of an 83-year-old patient who developed necrosis of all four limbs after a stay in the ICU for a urinary septic shock.
