The design of the standard Gamma Nail has changed.
This resulted in a shorter nail with only one distal locking
and with 4° medilolateral angle of curvature. This achieves
good intramedullary centering on the nail. But clinical experience
has showed that this design also has postoperative
diaphyseal femoral fractures. In a retrospective trial, we
study 111 fractures in geriatric patients treated by
Trochanteric Gamma Nail since May 1998 to December 1999.
The overall rate of diaphyseal fractures was 5,5%. Most of the
intraoperative and postoperative complications have decreased.
This could be explained by the fact that the surgical instruments,
the surgical technique and the experience have
improved. But we think that postoperative diaphyseal femoral
fractures are still present, mainly due to clinical and biomechanical
factors.
