The aim of this study was to review the results obtained with the different treatment alternatives in the treatment of complex fractures of the tibial pilon treated. Material and Methods. An observational, retrospective study, a series of 37 consecutive cases operated on between 2006 and 2007. In addition to demographic variables, we evaluated the type of fracture according to AO classification, treatment, functional results according to the rating scale of the Mutua de Accidentes de Trabajo y Enfermedades Profesionales de la Seguridad Social (FREMAP), radiology results by Burnell, Charnley, and employment outcome according to the Instituto Nacional de Salud. We evaluated the statistical correlation between different variables. Results. A total of 37 patients, mostly men with a mean age of 40 years, diagnosed with complex pilon fracture. The mechanism of production was the most frequent precipitation from heights, particularly construction workers. According to the AO classification, the majority (26/37) had type 43C, and of these, the variant C3 in 15 cases. The most commonly used treatment was a plate (19/37), followed by external fixation (18/37). As for the assessment work, 12 cases were cured without sequelae, 13 baremables sequelae, while 12 progressed to partial or total disability. According to FREMAP rating scale, the results were good or very good in 21 cases. There was a moderate significant correlation (p <0.05) between reduced and the results achieved (C. Pearson 0.413). Conclusion. Although there is no consensus as to the ideal treatment of these injuries, it is clear that combining external fixation and internal fixation with a plate had better outcomes.
