Ostheocondritis in the hand is a rare process. Only two cases were reported in the previous literature. In this report we present a 10 years old boy with pain and a few functional limitation in second metacarpal phalangeal joint for weeks. With the final diagnosis of osteochondritis dissecans we performed surgery. Intraoperatively the cartilage flap was removed and some drilling was realized. Before one year the patient was asymptomatic and we observed an improvement of articular surface.
