Professionalism is not a static structure within which we deliver medical care. Rather it can be viewed as a dynamic framework derived from the interaction of many forces. This paper deal with definition, evolution across the generations and teaching. This requires an institutionally accepted definition which then must be learned by both students and faculty. To improve the outlook and efficacy of modern physicians, the profession must adopt a new values framework to today's health care system. The force to blunt the corrupting power (commercialism) in medicine is professionalism. To finish, the idea of professionalism in 21st century professional practice in orthopaedic surgery is offered.
