The treatment of the unstable fractures of the trochanteric region of the femur in elder patients by means of the ondylo-cephalic nail of Kuntscher. Is considered 28 patients have been treated and controlled who have not been allowed to begin weight=bearing till the end of the fracture healing to avoid the penetration of the nail into the joint cavity through the femoral head or its distal migration. This series is compared with another one similar of patients who began weight-bearing soon after the osteosynthesis. The results prove the advantage of avoiding weight=bearing to obtain a correct bone healing without complications of the synthesis and the harmful effects of the precocious weight=bearing, which is the cause for proximal and distal migrations of the nail in more than half of the cases
